    The forms needed for sports clearance are found below. ALL forms must be submitted to the Nurse before the first day of tryouts.

    Required one time each school year.

    Athletic Physical Form

    Valid for 365 days. Required each year.

    The state of New Jersey has changed the Preparticipation Physical Exam forms required to participate in school athletics. This change became effective on July 1, 2024. HOWEVER, if your child had a physical examination prior to this date the forms previously used are still valid. 

     

    Preparticipation Physical Evaluation HISTORY FORM - This form is completed by the parent and athlete prior to the physical examination appointment and taken to the provider’s office. The provider reviews this and retains it in their office.


    Preparticipation Physical Evaluation  PHYSICAL EXAMINATION FORM - This form is completed by the provider and is retained in their office.


    Preparticipation Physical Evaluation MEDICAL ELIGIBILITY FORM - This form is completed by the provider and is returned to the school nurse (Mrs. Pape)
